Challenge Fund Application

Challenge Fund Sample Budget
Use this template to create a budget for a Student Challenge Fund application. All applications must include a budget in order to be considered for funding.
Travel, Inc. Registration
Use this form if you are being reimbursed for travel and need to complete a single trip authorization form to be approved to book your airfare through Travel, Inc.
Award Submission
Read about the various awards given by the John H. Martinson Honors Program and nominate yourself or a peer.
JHMP Photo Release
All incoming new students must complete a photo release form before participating in JMHP events and activities.
Event Request Form
Use this for requesting that the JMHP host an event. If you would like a non-JMHP event to be added to Jacket Journey, request that through the Suitable app.
Supplier Invoice Registration
Use this form if you are a retreat guide who needs to register as an external vendor in order to receive payment. Current GT employees do not need to register as vendors.
Spend Authorization
Use this form if you are a current GT employee and need to create a spend authorization to be approved for GT-sponsored travel.
Expense Report
Use this form if you are a current GT employee and need to file an expense report after returning from GT-sponsored travel.
JMHP Liability Waiver
All incoming new students must complete a liability waiver form before participating in JMHP events and activities.
